Complete the application form.
- Requires signature of medical professional.
- Anyone other than the applicant may witness the applicant’s ID information on the form.
- For more information, visit the state site.
Return the form online. You do not need to come into an office.
- If you have trouble submitting the form online, you may also fax it, mail it, or come in.
- If you choose to come into our office (not required), the person applying must come in, or a designated Power of Attorney.

All placard renewals can be completed online. Notes:

Renewals of permanent placards can be self-certified for the 1st and 2nd renewal (years 3 and 6).
The 3rd renewal (year 9) for a permanent placard requires a new application signed by a medical professional. Follow the new placard process above.
All other placard types (short-term temporary, temporary, and extended) require a new application signed by a medical professional every renewal cycle. Follow the new placard process above.
