Application Process

All new issuance of placards or plates require an Application Form (DR 2219) (PDF) completed in full and signed by a medical professional. Anyone other than the applicant may witness the applicant's ID information on the form.

Processing Application

Completed forms may be faxed, mailed, or processed in person. If the form is processed in person, the applicant must be present with Photo ID or representative must show Power of Attorney to receive the placard(s) over the counter. All other situations will result in the mailing of the placard(s).

Renewal Process

Renewal  notices are only sent out for records listed as permanent on the original application form.  These records are eligible to self-certify for the 1st and 2nd renewal (Years 3 & 6), but must have a New Application Form (DR 2219) (PDF) completed in full and signed by a medical professional for the 3rd renewal (Year 9).

All other record types (Short Term Temporary, Temporary, and Extended) require a New Application Form (DR 2219) (PDF) completed in full and signed by a medical professional every renewal cycle.
