Connect for Health Colorado is Colorado’s Health Insurance Marketplace, the only place to apply for financial assistance to help reduce your costs and get access to free, in-person help available statewide.
The Medicare Savings Program is available to eligible, low-income Colorado residents who currently receive Medicare.
In order to speak on the behalf of, gather any information about, the individual receiving benefits, you must get this Authorized Representative Form (PDF) signed and into the proper Human Services area.
