The Healthy Jeffco network (formerly called The Jeffco Community Health Improvement Network) is a group of multi-disciplinary organizations in Jefferson County working collectively to make Jefferson County a healthier place to live, learn, work and play. In order to improve the health and well-being of our community, organizations within the Network agree that we must align our activities and work together in a coordinated way.
Community Health Improvement Plan
The Network is using a collective impact framework to create change in our county. Collective impact is used for solving complex problems by engaging partners from multiple sectors who agree to work together to define the problem, set an agenda, align communication efforts and share data. Jefferson County Public Health is coordinating efforts of the network. Healthy Jeffco is the implementation of our Community Health Improvement Plan 2014 to 2018 (PDF) updated December 2016.
Healthy Jeffco is working to create system and policy level changes that make healthy choices easier for Jefferson County children and families. It is comprised of seven coalitions, or “CoINs” (Coalitions Integrated in the Network). Please view the Healthy Jeffco 2016 Report to the Community.
