Birth & Death Certificates

Jefferson County Public Health Vital Records issues birth and death certificates, as well as burial permits, as mandated by Colorado State law. The office compiles data pertinent to birth and death statistics for submission to the Colorado Department of Public Health and Environment. Jefferson County Public Health Vital Records is now able to issue birth certificates for any birth occurring in the State of Colorado from 1903 to present. These are official state issued certificates.

Birth Certificates

  • Birth certificate = $20
  • Each additional certificate of the same record ordered at the same time = $13
  • MasterCard, Visa, checks or cash may be used as payment.

NOTICE: On December 19, 2018, the Colorado Department of Public Health and Environment’s Board voted to allow more identifying options for birth certificates. There will now be four options: male, female, X and intersex. To make changes on a birth certificate, contact CDPHE [external link].

Death Certificates

  • Death certificate = $20
  • Each additional certificate of the same record ordered at the same time = $13
  • MasterCard, Visa, checks or cash may be used as payment.
  • Please note death certificates between 1985 to 1995 will take additional time to process.

Colorado Adoptees

Colorado Department of Health and Environment (CDPHE) began processing requests for original birth certificates for Colorado adult adoptees on January 4, 2016. These requests are only being handled by the Colorado Vital Records office. For more information and to obtain the request form visit the CDPHE website [external link], or call 303-692-2227.

Vital Records Requirements

Receive birth or death certificates by mail or wait at our office. A valid driver's license is required. If you do not have a valid driver's license, valid identification will be required. Marriage Licenses
