Civil union license requirements

Age requirements

Legal age is 18 for both people. If one party is 18 or older but under guardianship, a guardian consent form is required. Access the Guardian Consent Form (PDF) here.


Valid, legal identification for proof of age is required.

  • Driver’s license
  • Military ID
  • Passport
  • State issued ID
  • Government issued foreign ID (accompanied by certified translation when applicable)

Required information

  • Divorce/dissolution information (name of spouse/other party and date, place, and court where granted or dissolved)
  • Parents names and current address
  • Place of birth (City and State)
  • Social Security number

Social Security Number

Per Colorado Revised Statute 14-14-113, Social Security numbers are required on the civil union license application. If an applicant does not have a Social Security number, an affidavit provided by the clerk’s office must be signed.

  • Brother and sister, uncle and niece, or aunt and nephew, whether by half or whole blood.
  • One of the parties is still married to or in a civil union with another person.
  • One of the parties is under 18 years of age.
