Fees:
There is an administrative fee of $14.50 per vaccine. Service will not be denied due to inability to pay for any childhood vaccine, and fees may be waived. Fees for adults vary depending on the vaccine.
If you are in an HMO, you are encouraged to use your HMO services for your immunizations. If you prefer to come to the Public Health Department, you are responsible for the charges and for reporting your shot dates back to your HMO.
Required and Recommended Childhood Immunizations:
Diphtheria, Tetanus, Pertussis (DTaP); Polio; Haemophilus influenzae B (Hib); Rotavirus; Measles, Mumps, Rubella (MMR); Hepatitis A; Hepatitis B; Tetanus, Diphtheria, Pertussis (Tdap), or Tetanus, Diphtheria (Td); Human papillomavirus (HPV); Chicken Pox; Meningitis.
(Under Colorado law, parents may choose to have their children exempted from immunization requirements for medical, religious or personal reasons.)
- Children should be vaccinated at 2 months, 4 months, 6 months, 12-15 months, 4-6 years and 11-12 years.
Pneumococcal conjugate vaccine is required for all children under 5 years of age attending a licensed child care facility.
